Application Procedures
Residents of private, contracted (including the day care units attached to contracted residential care homes of the elderly) as well as self-financing residential care homes for the elderly and self-financing nursing homes can apply for our services in person or through following persons or channels:
- Caregivers
- Staff form the homes
- Staff from the Pilot Scheme on Multi-disciplinary Outreaching Support Teams for the Elderly
- Hospital Authority
- Visiting health team/Elderly health service (DH)
- Visiting medical practitioner for residential care homes (VMPS)
- Visiting medical officer (VMO)
- Others (such as social service units)

Withdrawal Procedures
Service users could apply for withdrawal of services directly;
or, when the resident’s treatment goals have been achieved or the service needs are out of the scope of the Team as determined by the Team’s professional assessment, after deliberation, the team could terminate services or make appropriate referral for users in need. The decision of the team shall be final.
